This survey has been designed to obtain feedback regarding the ratee's performance from individuals with whom s/he interacts on a regular basis.
The results of this survey will be used to help ratees identify how others perceive their work performance, including strengths and development areas, as well as how their behavior supports the achievement of company goals.
Your responses are completely confidential. They will not be associated with your name in any way and the reports will be structured to prevent anyone who reads them from guessing who you are.
